Consent Declaration (to be completed by Next of Kin)

I confirm that I am the next of kin for the named individual below and give my consent for them to participate in trips and outings organised by the Burnt Oak Nepalese Community (BONC).

I further confirm that I have disclosed all relevant medical conditions, care requirements, or other concerns that may affect their participation.

I am also satisfied with how BONC processes both my personal data and that of my senior relative in accordance with our privacy policy - Privacy policy

We regularly organise trips and outings for our senior members, some of whom may be elderly, vulnerable, or have specific medical conditions or care requirements.

To ensure their safety, wellbeing, and to meet our legal and safeguarding obligations, we require a Next of Kin Consent Form to be completed.
